Coventry Youth Football Concession (tax exempt), 981 Ridge Rd. Pottstown, Follow-up, October 27. Pass. Half and half was being held in ice that did not completely surround all sides of the container. Half and half above the level of the ice was at 48 degrees F, rather than 41 degrees F or below. Half and half was removed and replaced with a new container. Cold-holding procedures were discussed with employees on location, as well as using time logs as an alternative. Facility was using Steramine (quaternary ammonia) as the sanitizer, but did not have quat test strips on location to ensure the sanitizer is between 200-400 ppm. Within a week, provide proof of purchase (a picture or receipt) to show the correct test strips have been purchased.
Bloom Southern Kitchen, 123 Pottstown Pike, Chester Springs, October 25. Fail. Chicken breading is stored at room temperature and thrown out at the end of the night. This breading must be discarded at a minimum of every 4 hours. Containers must be time tracked. Provide proper time tracking sheet. Multiple meats were observed in reduced oxygen packaging. When discussing this with the CFM, ROP should not be done without a HACCP Plan. Discontinue ROP protein immediately. Contact [email protected] if you wish to continue. Observed discoloration on the drop chute and side paneling of the ice machine. Clean and sanitize within 48 hours. Maintain in a clean and sanitary state. The following non-food contact surfaces need to be cleaned: Shelving under the steam table; Floor underneath fryer and grill - accumulation of grease and food debris. Hot water at the bar handwash sink was not turned on at the time of inspection. Ensure the handwash sink at the bar has an adequate supply of hot water at all times. Correct immediately. Handwash sink in the kitchen was only able to be turned on by the valve on the pipe and not with the handles on the faucet. Contact a plumber immediately and maintain the sink in good repair. Both handwash sinks upstairs were not stocked with soap and paper towels. Ensure all handwash sinks are stocked with soap and paper towels to ensure proper handwashing. Correct immediately. Fryer basket was observed to have broken wiring. Replace basket immediately. Steam table was observed to have a slow leak at the time of inspection. CFM indicated that it had been serviced and was continuing to be monitored. Continue to monitor the table and ensure it does not get worse.

Replace test strips with a full-size pack and advise all food workers who do dishwashing to use the test strips daily to ensure the quat sanitizer is between 200-400 ppm. Just Mom's Llc, 1050 N. Manor Rd. Honey Brook, Complaint, October 25. Fail. Food Facility Person in Charge was not able to provide documentation that multiple foods (jams, jellies, pickles, juices, etc) were sourced from or prepared in an approved source. All unapproved items were pulled from sale. In the outdoor walk in, foods and food containers were stored directly on the floor, rather than 6 inches off the floor as required. Move all foods and food containers 6 inches off the floor. Food facility is preparing and packaging juice without a HACCP plan or any means of providing for a 5 log reduction of pathogens, or alternatively providing a consumer advisory or warning label on the juice. Juices removed from sale. Contact for information on obtaining a variance and writing a HACCP plan. In the ice cream area, the lowboy worktop refrigerator was holding foods at 47.6 degrees F, rather than 41 degrees F or below as required. The following were discarded from the unit: 9 containers ice cream mix, 2 containers whipped topping, 1 container cannoli cream, 1 can pineapple, 1 can peaches No foods may be stored in this unit until it is repaired and verified that it is holding foods at 41 degrees F or below. Food Facility did not obtain a variance from the Department before preparing foods preserved by making jams, jellies, or pickling, which are specialized processing methods. Items removed from sale. Contact for information on obtaining a variance and writing a HACCP plan. Single use aluminum cans being used for food storage after being opened. These cans are not designed to store foods after being opened, and this could contaminate the food. After a can of food has been opened, place any unused portion of food in a durable, food-grade reusable container for storage. Electro-freeze soft serve machine is not being dissembled, cleaned, and sanitized daily as per manufacturer's specifications. Dissemble, clean, and sanitize the soft serve machine daily. Multiple pieces of equipment have a buildup of grease and grime. Clean all equipment, with extra attention to the following: Handles of equipment; Paper towel dispensers; Hand sinks; The drip tray of drink dispensing equipment. The handwash sink in the pizza prep area has a very low flow and the cold water is not working. Repair sink to dispense hot and cold water easily. Soap was not available at any of the handwash sinks in the kitchen area. Hand sinks must be stocked with soap and paper towels at all times. Corrected on site: Soap brought to the hand sinks. The 3-compartment sink was being used as a handwash sink, which could contaminate food equipment with germs from hands. Clean and sanitize all compartments of the 3-compartment sink before washing dishes. Employees advised on location to only wash hands in the handwashing sink. Floors, shelves, work tops, and surfaces throughout the facility have a buildup of grease and grime and need to be thoroughly cleaned, especially underneath and behind equipment. Boxes of liquid ant bait, a restricted use pesticide, were stored on the same shelf as foods, and above more foods. Restricted use pesticides may not be used inside food facilities, and any toxic materials must be stored separately from foods. Pesticides not currently in use. Boxes moved to a shelf away from foods.

Foods were covered by the end of the inspection. Shrimp was being thawed in the 3 compartment sink and not the food prep sink. All food prep should be done in the designated prep sink. Shrimp was moved to the food prep sink and 3 compartment sink was cleaned and sanitized. Employee food was not being stored in a designated area for employees. Make sure all food for employees are sectioned in an area for employees only. Containers of chicken were not date marked. Date mark TCS foods on the day they were made and discard after 7 days. Food facility inspection indicates evidence of rodent activity behind the chest freezer near the bathroom, but facility does not have a pest control program. Facility needs to contact a certified pest control specialist within 24 hours.

Ram's Head Diner, 907 S. High St. West Chester, October 24. Fail. Employee observed handling dirty dishes then handling clean dishes without washing hands in between, risking contamination. Train staff on when to wash hands per food code and comply. Open employee beverage containers were observed throughout the kitchen. To prevent contamination of food/equipment, only lidded cups may be used and beverages must be stored in a sanitary manner. In top of bain marie along mainline, raw eggs found stored above ready to eat foods such as pickles, risking cross-contamination. Raw and under-cooked animal proteins must be stored below all cooked and ready-to-eat foods. Rearrange foods. Many foods observed uncovered in refrigerators and freezers. Keep foods covered. Along expo line, label squeeze bottles with the common food name of the contents within. In dessert station, label bulk ingredient bins with the common name of the contents within. Loose croissants stored in plastic "take-out" style grocery bag, risking contamination. These bags are not made for direct contact of ready to eat foods. Use food grade bags for storage needs. Open container of BBQ sauce found stored at room temperature although label states "refrigerate after opening." Discard sauce. When new bottle is opened, place in refrigerator. Time/Temperature Controlled for Safety (TCS) ready-to-eat food prepared in the food facility and held for more than 24 hours, located in the walk-in refrigerator, are not being date marked. Provide date labels and use foods within 7 days. Provide thermometer inside the small reach-in refrigerator along the mainline. Peeling caulk observed in hood canopies. Remove damaged caulk and apply new caulk. Clean & sanitize the table-mounted can opener. Clean the exterior/sides of the fryer and flat grill. Lid to exterior grease drum flipped open. Keep closed. Clean the floor in the walk-in freezer.
The Hungry Pilot, 1 Earhart Dr. Suite 3, Coatesville, October 24. Pass. Large prep refrigerator located on cooking line was observed at 57 degrees F rather than 38 degrees F or below. All potentially hazardous food was discarded during the inspection. It was discovered that the refrigerator was accidently unplugged, probably from cleaning the previous night. Refrigerator was plugged in and dropped to 41 degrees F while on-site. Violation was corrected. Remember to check refrigeration temperatures daily and throughout the day. Dishwasher in bar area wasn't dispensing chlorine during the sanitizing cycle. Chlorine sanitizing solution should have a chlorine concentration between 50-100 ppm. Do not use until serviced. Kitchen dishwasher is operating properly and may be used. Correct immediately. Outdoor storage area for refuse does not have non-absorbent flooring, such as concrete or asphalt. Correct within 30 days.

Continue to monitor temperatures. Food facility does not have Chlorine test strips for the 1st floor kitchenette dishwasher. Facility will be getting new dishwashing units soon. Ensure those are also stocked with high temperature measuring devices/strips and appropriate sanitizer test strips (chlorine, quat, sink & surface). Order test strips immediately. Thoroughly clean the ice machine. Build up was observed on the drop plate. Ensure the whole unit including the maker components are in a clean and sanitary condition. Clean immediately. Clean the following non-food contact surfaces: Clean floor underneath the cookline; Clean the shelving and the walls of the Hoshizaki fridge next to the fryer; Clean the coffee/drink area of the first floor kitchenette. Clean within 24 hours. No Chester County CFM on staff. Sous Chef and Executive Chef both have valid Food Manager certificates. Apply for a CCHD CFM license with 2 weeks. Application will be sent via email.

PJ Whelihan's Pub And Restaurant, 853 E. Lancaster Ave. Downingtown, October 23. Fail. Observed a piece of dark blue plastic in the Ice Bin near the food prep area. All ice was ordered discarded on site and facility complied. Ensure all ice is protected from contamination at all times and that employees monitor the ice. Pans of cooked chicken wings in the walk in refrigerator were stored open with no covering. Correct today. Thawing turkeys were stored above pans of ready to eat chicken in the walk in. The cooked chicken was removed on site. Comply every day by keeping raw meats stored below all ready to eat foods. A dry food ingredient storage containers, in the Storage Room, was not labeled with the common name of the food. Correct today. The scoop for the flour bin in the Storage Room was found stored on top of the bin. This exposes the scoop to contamination. It must be stored inside the bin with the handle up. A small can of Chipotle Peppers was observed opened and covered with plastic wrap in the walk in. Once cans are opened all food must be used or placed in a non-corrosive container. This was corrected on site. Observed a sanitizer bucket next to pans of food on a cart. This could cause contamination. The food was moved on site. Two (2) five (5) gallon containers of Chili was not cooled from 135 degrees F to 70 degrees F in 2 hours and/or from 135 degrees F to 41 degrees F within 6 hours, after preparation. This was confirmed since the labels on the Chili stated it was made yesterday at 5:30 p.m. and the internal temperature of the Chili was found at 56 degrees F at 11 a.m. this morning. The containers were ordered discarded on site and facility complied. Must use shallow pans to cool foods within the time frames stated above and/or purchase a few ice wands to cool the chili. Correct today and comply everyday. Observed 5 gallon containers of Chili being cooled in the walk-in cooler, which is not a proper cooling method. Correct today and comply everyday. A large container of Ranch Dressing was held at 47 degrees F, in an ice bath on the rail in the Kitchen, rather than 41 degrees F or below as required. Not enough ice was being used to entirely cover the dressing in the bowl. It was discarded on site. Must bury the container on all sides and the ice must be as high as the food level. Comply everyday. Time/Temperature Controlled for Safety (TCS) ready-to-eat food-Trays of Cooked Chicken Wings- prepared in the food facility and held for more than 24 hours, located in the Walk In were not date marked. Correct immediately and comply everyday. Walk In Freezer: Repair the heater mechanism around the Door/doorway; it is frosted up with ice. Repair the floor panel that is raised up-repair to render the floor smooth easily cleanable and good repair. Correct the above within 14 days. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration of Quaternary Ammonia Sanitizer at both Bars. Supply QT 10 test kits within 7 days. Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was 200 ppm, and not 50-100 ppm as required. This machine cannot be used until it is repaired. Eco Lab was called on site and immediately came and repaired the machine. Chlorine was then found at 100 ppm. The following need cleaned: The plastic cart in the prep area-correct today; BEER Walk In: Sweep and clean the floor; Walk In FREEZER: Clean the floor; Remove all rust inside and outside of the hood canopy over the Dish Washer and clean it; Remove all black tape from the pre-rise faucet at the dish washer and clean the faucet handles TODAY. Correct within 7 days. Observed clean food equipment stored while wet, and not allowing time for draining and/or air-drying. Correct today and comply everyday. Repair the following: The drain under the food prep sink-the drain is leaking; The drains under the Wash and Rinse Sinks of the 3 bay sink-they leak. Correct within 7 days. The quarry tile floor in front of the main cooking line, behind the cooking line, the dish washing area, and the Mop Sink Room. Correct within 3 months. BOTH BARS: Install soap dispensers to all hand sinks. Correct within 48 hours. BOTH BARS: Install paper towel dispensers to fit the brown rolled paper towels. Correct within 48 hours. Aramark @ Chester County Prison, 501 S. Wawaset Rd. West Chester, Follow-up, October 22. Fail. Dietary Area - steam table has not been repaired. No foods were being stored in the unit at the time of inspection, but it was reportedly still being used. Unit was to be repaired today, but work had not begun by end of inspection. Ensure daily temp checks are done at this stationd and recorded. ODR dishwasher - while it did reach 160 degrees F at the dish surface, the cycle still does not seem to trigger properly. Service this machine. Lack of hot water to the kitchen is hindering the use of the main dishwasher. The machine struggles to maintain correct water temps to ensure all dish surfaces reach 160 degrees F. Staff today said they ran the thermometer several times and while most of the time it read 160 degrees F+, there were runs that it did not reach temp. An additional hot water heater, additional or larger booster heater is needed to ensure this machine works properly. Ensuring that trays are properly sanitized is critical. Install upgrades or new equipment within 14 days. Lack of hot water was noted in the ODR again. Water had to run for several minutes and a valve checked in main kitchen. An additional heater is needed for the ODR to ensure it is always supplied with hot water.
Avon Grove High School, 10 Waltman Way, West Grove, October 22. Pass. The food-contact surfaces of equipment and utensils did not reach 160 degrees F during the sanitizing cycle of the high temperature dishwasher as evidenced by the failure of the temperature indicator. The hot water used for sanitizing in the mechanical warewashing machine did not reach 180 degrees F (or 165 degrees F for stationary rack).

Corrected on site: Onions moved off the floor. In the kitchen, the two True refrigerators had inaccurate outside thermometers, and either had missing or inaccurate thermometers on the inside. Corrected on site: New, working thermometers were placed inside both units. The following food-contact equipment need to be cleaned: Both ice bins at the bar need to be cleaned and sanitized before they are used; The ice machine in the back needs to be cleaned and sanitized, especially along the plastic ice chute. The outside dumpster has holes rusted through the front. Dumpster company was called during the inspection to ask them to replace the dumpster. Soap was not initially available at the bar hand sink. Corrected on site: Soap was brought to the hand sink. Paper towels were not initially available within arm's reach of the bar and kitchen hand sinks. Staff provided covering for these units that will be provided during non-rush operations typically between 2-4:30 p.m. Large plastic containers on top of convection oven observed cracked/chipped and strewn together with duck tape. Staff discarded containers and informed to discard all cracked/chipped/damaged containers. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dust, debris, crumb, or splash; Top of low boy unit on cook line, below range and flat top griddle; Floor under Fryers (2); Clean out mop sink and organize mope sink closet. Clean the top of the exterior oil bin and pressure wash pavement where splash and overflow has occurred. Excessive empty kegs observed in lower level room. Contact distributer and have kegs returned.

Casa Herrera, 2755 E. Lincoln Highway, Thorndale, October 22. Pass. BAR: The 3 bay sink was not set up correctly. From right to left it was: Wash, Sanitize, Rinse. It must be WASH, RINSE SANITIZE. This was corrected and explained on site. Comply every day. BAR: The quaternary ammonium concentration in the sanitizing solution of the 3-bay ware wash sink was 100 ppm, rather than 200 ppm as stated on the manufacturers use directions. This was explained and corrected on site. Observed the re-use of many white buckets and square white plastic containers. These containers are made for one time use ONLY-and may not be approved for food contact. Stop using these containers today. Supply food grade containers within 7 days. The grout has worn away around the quarry tiles on the floor in the Dish Washing room, allowing for food and water to collect. Re-grout the floor tiles to render a smooth and easily cleanable surface within 6 months. CFM Application and Fee for County Certificate never received. This was to have been received by 1/11/24. The Application and Fee were received on site today. Please post her CFM Certificate in public view once received.

Great China, 929 S. High St. West Chester, October 22. Fail. Fresh broccoli crowns must be washed prior to cutting. After cutting, broccoli must be placed in a clean, food grade container. Do not put washed broccoli back into the original shipment box and this box is not clean and risks contaminating the food. Repair the leak from the faucet of the 3-bay sink. In employee restroom, clean walls and apply a fresh coat of paint. After walls have been painted, re-seal/re-caulk the sink to the wall. In employee restroom, clean the sink & toilet.
Hank's Place, 201 Birch St. Kennett Square, October 22. Pass. Observed a bag of wings and another pan of food stacked directly on top of a pan of fish that was only covered by plastic. Cannot stack foods on top of each other if foods are not covered with solid lids and sealed tightly. Hood filters were cleaned but were not placed back into hood before cooking began today. Smoke and grease were going directly into the upper hood canopy. Filters must be in place when cooking is done. Install filters today. The handwash sink located in the restrooms not have water at a temperature of at least 85 degrees F. Max temp after running for several minutes was 66 degrees F. Increase temp to 85-110 degrees F.

Re-route waste line outside of three-compartment sink, with an indirect waste line. Correct within 7 days.

Oops Donut, 235 Lancaster Ave. Malvern, October 22. Pass. Observed raw eggs stored above ready-to-eat foods in the 2-door refrigerator. Eggs were moved to prevent potential dripping onto food if cracked/broken. Observed scoop stored in ice in the ice machine. Scoop was moved to a hanger by person in charge. Observed food stored in open in cans in the undercounter cooler and 2-door refrigerator. Open cans can become rusted. After opening a can, move food to a food storage container. No hand soap available at the hand wash sink next to the ice machine. Person in charge brought hand soap.
Sake Hana, 801-805 Gap Newport Pike, Avondale, Follow-up, October 22. Fail. A dented canned item was observed on the shelf. The dented canned was discarded. Discard the dented canned items. Garlic and oil mixture was at 62 degrees F inside the top of the cold holding unit across the cook line. The garlic and oil mixture were discarded on site. Ensure the garlic and oil mixture is kept in the refrigerator. The facility had all items date marked, however, was found to have some inconsistency. For example, the facility is writing the expiration dates on items, and today an item had an expiration date mark of 10/31. That is more than 7 days if the items were prepared today. CCHD recommends the facility dates the item on the day it was prepared and discard or use by within 7 days of that date. Retrain employees. Walk in cooler temperature is rising and dropping rapidly. The temperature was recorded at 41 degrees F. The food temperatures was in proper temperatures. The facility needs to have the walk-in cooler serviced by a professional and provide the documents to the Department. Monitor the temperatures of the walk-in cooler and keep a log to ensure that it is maintaining 41 degrees F or below. The Department needs a documented work order, please indicate on the work order what the temperature of the walk-in cooler is at when the technician departs. The department recommends setting their walk in cooler temperature to 34–38-degree F range. Some mouse droppings were observed on some canned items. The cans must be washed, rinsed, and sanitize before opening them. Observed some mouse droppings on the floor behind a fridge in the kitchen, behind the chest freezer, and in the dry storage area. The facility has been receiving professional pest control treatment and had the proper documentation. The facility must continue to do the following: Certified pest control operator must continue service; Submit copies of reports to Department promptly after each service; Maintain facility surfaces, plumbing and equipment in clean condition and good repair; Inspect incoming shipments for vectors; Keep all foods covered in storage; Remove any/all mice droppings and disinfect the affected area. All single-use articles must be discarded if they are not kept in the protective wrapping. A Restricted use pesticide is being applied in the food facility by a non-certified applicator. Ensure all pesticides are being applied by a certified applicator. Today the cockroach bait was discarded on site. Only a certified pest control applicator can apply pesticides in the food facility. Food facility unable to provide documentation for procedures, monitoring of critical control points, and corrective actions in the HACCP plan submitted to and approved by the Department. Facility needs to create procedures and a HACCP plan for the ph of sushi rice. Facility is actively working on this HACCP Plan.
